This form can only be filled out by Kol Emeth members for up to two (2) immediate family members (ie, adult children, parents) who they are bringing as their guests.
If you need tickets for non-immediate family members (ie, cousins, aunts, uncles, etc), please scroll down to the second half of this form. Each ticket will be $350 per individual, and $700 per family.
If you are not a member, please refer back to our Non-Member form.

Guest tickets (including immediate family members' tickets) will be available at will call. They will not be included in our High Holiday mailing.
